Microsoft JMRP setup form is broken

Hello,

Our organisation try to solve mails delivery problems with MS domain (hotmail, live, msn, etc.). We successfully enroll MS SNDS program (IP 91.216.110.28), and would like to do so with the Junk Email Reporting Program (JMRP), but the setup form seems broken at: https://sendersupport.olc.protection.outlook.com/snds/Jmrp

The field "Complaint format" is not selectable and submiting the form return the error: "Select one of the complaint format options".
